Your “homecare plan” will
fit you - A
primary objective of homecare is to enable the patient
to become well, more quickly and completely, by recovering
in the comfort and familiarity of home. Getting well – quickly
and fully – requires a home care plan that provides
patients with the complete range of services and care,
which are needed and entitled.
While this seems simple enough, too often it is not.
Many homecare agencies and even physicians are not
well versed in the types of care to which their patients
are entitled. Our professionals will ensure that all “care
options” are explored and that the plan of treatment
we recommend is both comprehensive and fits the individual
needs of each patient.
You will fully understand your “treatment plan” - We will make
sure all patients are given a thorough understanding of their plan for home
care treatment – both in person and in writing. More importantly, we
will answer or find answers to all the questions a patient may have.
Home care has become increasingly complex, especially
for Medicare and Medicaid eligible patients. It is
not easy for patients suffering from serious illnesses
or injuries to understand their eligibility and the
types of care, which may apply. Our professionals will
see to it that no patient question goes unanswered.
You will see the same team - Nothing is more frustrating
to a patient than seeing an ever-changing parade of
faces. HSG organizes its professionals into teams by
physician office and case manager. As much as humanly
possible, patients of the same physician will see the
same HSG professionals throughout their home care recovery
period. This approach maximizes continuity, communications
and confidence.
We will be on time - No need to explain. Our professionals
will be on time and stay until the patient’s
care is completed. If they don’t, we want to
know and changes will be made.
We will check in on “off” days
if you want us to - Most home care plans
provide for visits several times a week – but
not every day. On those days when a visit is not
scheduled, we will arrange
to check in by phone at the patient’s request.
In most cases, everything will be fine, but in the
event a patient needs some extra attention, we will
be there.
You will have 24/7 access to our professionals - Homecare
Solutions Group provides access to its home care professionals
on a 24 hour, 7-day per week basis. Emergency contact
numbers are made available to all patients, physicians
and case managers. HSG professionals are on call at
all times.
Your physician will know how you are doing - All HSG
patients receive home care treatment in accordance
with a “plan of care”, which has been authorized
by their personal or attending physician. This plan
of care will “spell out” those changes
in patient condition, which are to be reported to the
physician promptly. HSG is organized to deliver physician
updates by telephone, fax and email. In addition to
immediate updates on changes in patient condition,
we provide weekly electronic updates on every patient,
regardless of status.
Every patient who receives care at home from
an HSG professional should expect these promises
to be kept – every
one, every time.
