We will follow Diagnosis-Specific Protocols – In
the age of “specialties”, it has become
increasingly probable that most patients of the same
physician will require similar plans for home care.
HSG is committed to working with physicians and case
managers to design “diagnosis-specific” protocols.
These protocols will reflect the best thinking of the
physician responsible, the case manager, our home care
professionals and other physician’s with whom
we work. They will be in full compliance with Medicare,
Medicaid or other related managed care guidelines.
MOST IMPORTANTLY, all HSG professionals assigned to
patients for whom a diagnosis-specific protocol is
authorized will be fully trained and closely supervised
to ensure full compliance. Likewise, our “reports” to
the physician will be aligned directly with the DSP.
We will make every effort to assign the same
team of professionals – In order to apply diagnostic-specific
protocols consistently and maintain patient confidence,
HSG commits to providing the same team of professionals
throughout each episode and to all patients within
a “physician practice”.
This accomplishment requires highly effective scheduling
of professionals and limits our flexibility. Nevertheless,
it is so clearly in the best interest of patients and
the physicians overseeing them that it is one of our
most important “promises”.
We will provide immediate notification of
changes – The
use of diagnostic-specific protocols facilitates the
establishment and recognition of clear parameters for
physician notification. HSG professionals are trained,
supported and supervised to provide immediate physician
notification when patient condition moves outside acceptable
parameters.
We will eliminate unnecessary interruptions – Few,
if any, physicians or case managers have time to field
superfluous phone calls concerning their patient’s
home care needs. And yet, it seems to be the number
one complaint about home care agencies.
Unnecessary interruptions occur for several reasons.
Too often Plans of Care are poorly conceived and inconsistent.
By emphasizing diagnosis-specific protocols and organizing
our professional teams along physician office lines,
HSG nurses become highly familiar with patient recovery
plans. The results are greater self-sufficiency and
fewer unnecessary questions. Additionally we provide
one of the highest levels of field supervision for
our professionals, which enable questions to be answered
before they require a bothersome call to the physician’s
office.
We will support physicians who wish to qualify
for care plan oversight – Medicare recognizes that
physicians play a critical role in overseeing the recovery
of their patients during the home care phase. HSG is
committed to helping physicians receive the care plan
oversight reimbursement to which they are entitled.
We will employ technology to make those who
assist the physicians and the case managers as effective
and
efficient as possible – In each case, we will
work to provide information and communication in the
way that best accommodates the needs and preferences
of individual physician offices and case managers.
